To help you make the most of your medical plan, both Anthem and Kaiser offer multiple health management and wellness resources to members.

Anthem

Concierge Cancer Care

A cancer diagnosis can be life-changing and overwhelming. If you are diagnosed with cancer and are enrolled in any Anthem plan (Blue Open Access POS, Blue Open Access HMO or Blue Open Access HSA), you have access to Concierge Cancer Care.

Concierge Cancer Care provides:

  • Guidance: The right connections for every moment, from diagnosis through recovery.
  • Innovation: Technology for transformative impact on outcomes, including Tytocare telehealth platform.
  • Partnerships: Collaborations with leading cancer experts and world-class institutions and centers of excellence.

Lark Diabetes Prevention

If you’re enrolled in any Anthem plan (Blue Open Access POS, Blue Open Access HMO or Blue Open Access HSA), you have access to the Lark Diabetes Prevention Program.

Available at no cost to you, Lark provides:

  • Wireless fitness tracker and smart scale
  • Personalized health coaching
  • Compassionate support

Transform Diabetes Care and Hypertension

If you enroll in the Blue Open Access POS or Blue Open Access HMO plan, you automatically are eligible for the Transform Diabetes Care and Hypertension program through CVS Caremark. The program provides you with blood glucose meter and supplies, and a home blood pressure monitor, all at no cost to you.

If you (or a covered dependent) have a diabetes diagnosis, you may receive an outreach communication from a Certified Diabetes Educator. These professionals are either Registered Nurses or Registered Dieticians, and they will work with you to help monitor and control your diabetes.

ConditionCare for Chronic Conditions

If you or a family member has asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), type 1 or 2 diabetes, or heart failure, Anthem offers this no-cost health and wellness program, which provides:

  • 24/7, toll-free phone access to nurses who can answer health questions.
  • Support from nurse care managers, dietitians and other health care professionals to help you reach your health goals.
  • Educational guides, electronic newsletters and tools to help you learn more about your condition(s).

MyHealth Coach

Experienced health educators are here to help you and your covered family members make healthy choices and locate community resources. For example, MyHealth Coach can help you take steps to eat healthier, be more active, feel less stressed or nervous, quit smoking, and more!

DeKalb County Employee Health Center
The DeKalb County Employee Health Center provides comprehensive primary health care at little to no cost for DeKalb County employees and early retirees enrolled in an Anthem medical plan.

Kaiser

Comprehensive Care for Chronic Conditions

Kaiser Permanente’s integrated, patient-centered approach to health care and disease management programs supports members with chronic conditions such as asthma, cancer, depression, diabetes, hypertension, weight management and many others.

Omada Diabetes Prevention

If you meet eligibility requirements, Omada Health provides you with a personalized program offering up to $650 worth of tools and support, including secure, 24/7 access to:

  • An Omada health coach to keep you on track
  • A wireless smart scale to monitor progress (and keep for good)
  • A personalized dashboard to guide your journey
  • An online community tailored to your interests

The program is designed to help you meet your health goals, whether that’s losing weight or improving your overall health. Get started at Omada Health.

Wellness Program and Coaching

You and your covered spouse can each earn a $250 wellness incentive — an HSA contribution or reward card that can be used anywhere when you complete the Kaiser Wellness Program. There are just five steps to complete by June 30, 2024:

  1. Accept the Wellness Program Agreement.
  2. Take a 10-minute Total Health Assessment
  3. Get a biometric screening at a Kaiser Permanente medical office.
  4. Make sure you’re current with all recommended age and gender appropriate preventive screenings for breast, cervical, or colorectal cancer.
  5. Complete one online Healthy Lifestyle Program or partner with a coach through Wellness Coaching by Phone.
